The Diabetes Care Programme is a comprehensive care plan that provides cover for the tests required for the management of diabetes as well as your other chronic conditions.

Aim of the Diabetic Care Programme

The aim of the Diabetic Care Programme is to form a partnership between you, your nominated Premier Plus GP and Bonitas to effectively manage your diabetes.

Benefits of the Diabetes Care Programme

All Bonitas members with diabetes will have automatic access to the programme and its benefits once they have been diagnosed with diabetes.

The Diabetes Care Programme is based on clinical and lifestyle guidelines. Through the programme, you and your nominated Premier Plus GP will work together to develop a personalised care plan that meets your individual needs.

The programme includes:

• Regular check-ups with your nominated Premier Plus GP
• Cover for the tests required for the management of your diabetes, including blood glucose testing, HbA1c testing, and lipid profiles
• Access to diabetes educators and other healthcare professionals
• Support and education on diabetes management
• Access to the Bonitas Diabetes Helpline

The implementation of this updated guideline as one of the tools available to manage diabetes will, with the support of healthcare professionals, empower you to take control of your diabetes and live a healthy and fulfilling life.
